The self-referral form is open from 9.00am on Monday until 4.30pm on Tuesday, and from 9.00am to 4.30pm on Wednesday, Thursday and Friday. The form remains accessible overnight on Monday to provide flexibility for those who may prefer to complete it outside of normal working hours.
Please allow at least 15 minutes to complete the self-referral form and be mindful of the closing times above when starting your referral. If the form is due to close shortly, you may wish to wait until the next opening period to ensure you have enough time to complete and submit the form.
Once you have submitted your self-referral, we may contact you at any point during the working week, so please check your phone and emails regularly.
