The self-referral form is open from 9a.m. Mondays until 5p.m. Wednesdays which means that it is available overnight Mondays and Tuesdays. We will pause the form on Thursdays and Fridays to allow our staff time to read through the information provided and schedule appointments. Please note that once you have completed the self-referral we may be in touch with you at any time in the working week so please do check your phone and emails regularly.
